Collaborates with the interdisciplinary team, the
patient/family/caregiver, and the patient' s primary care provider
to develop a holistic plan of care focused on achieving the
patient/family physical therapy goals.
Instructs, coaches, guides, and intervenes to support the patient
and family in achieving the best possible outcomes while remaining
in the communities they value.
Promotes optimal health, well-being, and quality of life of home
health patients, and their families by providing skilled,
intermittent physical therapy in the patient' s home.
Practices independently in the patient home using excellent,
evidence-based practice for assessment, clinical decision making,
clinical practice, and coordination of care across the

health therapy services, when nursing is either not involved in the
case or physical therapy is the primary patient need.
Assesses the patient and collaborates with the interdisciplinary
team and primary care provider to develop the patient-centered plan

Completes accurate clinical documentation to effectively support
service provided within specified timeframes
Provides physical therapy to pediatric, adolescent, adult and
geriatric patients based on principles of growth and development

Provides instruction to the patient/caregiver and revises the
treatment plan as appropriate.
Maintains communication regarding patients with the case manager
and other disciplines via care conferences or other appropriate
Reports acute medical problems and condition changes to the primary
Provides supervision to Physical Therapy Assistants, Home Health
Aides, and students in the clinical setting.
Completes regulatory required assessments within specified
