We’re hiring an Ambulatory Clinical Pharmacist —a pharmacist who will play a hands-on role in elevating patient care. In this dynamic, patient?centered position, you’ll build meaningful relationships with patients and providers while delivering high?quality medication management across diverse disease states. Working remotely alongside an engaged clinical team, you’ll conduct virtual assessments, optimize therapy, resolve medication?related problems, and champion evidence?based care. Location: Orange, CA Salary: An employee in this position who performs the job duties in California can expect a salary rate between $150,000 and $190,000 (Hourly Equivalent: Approximately $72.12 - $91.35 per hour, based on a 40-hour workweek.) plus bonus pursuant to the terms of an any bonus plan if applicable. The actual salary will depend on experience, geographic location, and other factors permitted by law. Partner with physicians, specialists, and interdisciplinary teams to deliver individualized care plans and seamless medication coordination. Monitor therapy effectiveness and safety, identify optimization and cost?saving opportunities, and support quality?improvement and REMS initiatives. Streamline operations by supporting complex prior authorizations, improving workflows, educating patients, and maintaining ongoing clinical development. What You Need: Hold a Doctor of Pharmacy (PharmD) Must be licensed pharmacist in CA Advanced practice pharmacist in CA, by meeting two of these three criteria listed below: Possession of a current certification in relevant area of practice (e.g. Board Certification) Completed a postgraduate residency earned in the United States through an accredited postgraduate institution. Provided 1,500 hours of clinical experience under a collaborative practice agreement or protocol to patients within 10 years of application, where clinical experience includes initiating, adjusting, modifying or discontinuing drug therapy of patients. 3 years' experience as a clinical pharmacist or completion of PGY2 Strong communication, organization, and tech skills Empathy, integrity, and a team-first mindset California residents employed by or applying for work at Shields have certain privacy rights.
