Job Description:

A company is looking for a Supervisor, Care Management - LTSS.
Key Responsibilities

Supervises the care coordination team for long-term care members, addressing escalations and care management issues
Monitors compliance with documentation requirements and assigns caseloads based on state regulations and team experience
Evaluates team performance, provides coaching, and assists with onboarding and training of new team members

Required Qualifications

Bachelor's degree required
4+ years of related experience or equivalent experience
Experience in care management or related field preferred
Knowledge of federal and state regulations regarding long-term care
Experience in team leadership and performance evaluation
